ATTENTION ALL FPC PATIENTS
Information about COVID-19 and How it affects your care
Your safety is our top priority. During the COVID-19 pandemic we continue to ensure that every visit you have at FPC is safe. We have implemented some changes to the way patients typically access our care.
Beginning June 15th 2020, new patient assessments will take place in our office. Beginning July 13th 2020, established patient visits will resume in our office.
Do all visitors need to wear a mask? Yes, we ask that all patients with appointments in office please wear a mask if you have one. If you don’t have one, we can provide a mask for you.
Can family members/friends attend my appointment with me? Until further notice only persons with scheduled appointments will be allowed in the office. Exceptions would be minor patients under age 18, adult patients who have a legal guardian, and clients who reside in group homes/adult family homes. Those clients may have only ONE caregiver/guardian attend the appointment in office.
What if I am not comfortable coming into the office for an appointment? If you do not feel comfortable attending In person appointments, please call our office at 262-725-4426 to make arrangements for a virtual appointment. We will attempt to accommodate you as best as possible.
