Important Information About Your Care:
Medication refills
If you are in need of a medication refill please contact your pharmacy or request a refill through our patient portal(MYIO) from your provider. If you forgot your username or need to reset your patient portal password, please contact our office for assistance. Please note—if you do not have an upcoming appointment scheduled with our office you will need to call 262-725-4426 and schedule prior to having any refills sent to your pharmacy.
Provider questions
The fastest way to contact your provider with a question is through the patient portal (MYIO). If you have forgotten your username or need to reset your patient portal password, please contact our office for assistance.
Please note
- To ensure that you do not experience any interruptions in your medication, we kindly ask that you request refills 5-7 days before running out of your prescription. This allows us adequate time to process your request, contact your pharmacy if needed, and address any necessary follow-up. Please allow up to 3 business days for your request to be completed.
- To ensure timely and accurate processing of your prescription requests, we ask that you follow the guidelines below:
- Stimulant Prescriptions: Please submit all stimulant prescription requests directly to your provider through the MYIO portal. This allows us to manage these requests in compliance with current regulations and ensure your treatment plan is maintained.
- All Other Prescriptions: For all other medication refill requests, please contact your pharmacy directly. They will communicate with our office as needed to process your refill.
- Any messages sent to your provider after 3:00pm will be responded to on the next business day your provider is in the office.
- If there is a regional and/or national shortage of a medication you are being prescribed—it is the responsibility of the patient to contact alternate pharmacies to check their stock if you would like the prescription transferred.
- Please allow up to 7-10 business days for requested paperwork (disability, FMLA, etc.) completion and note that an appointment may need to be scheduled for paperwork to be completed.
- If you arrive more than 5 minutes after your scheduled appointment time, we may ask you to wait for the next available appointment slot or you will be given the option to reschedule your appointment to another day.
